Management

In the management of isoimmunized pregnancies the aim is to predict whether the fetus is severely affected and to correct the fetal anemia by intrauterine blood transfusion. In patients with previous severely affected pregnancies and in those with maternal antibody levels greater than 15 IU/mL (titer of 1 in 128 or more), the fetus should be assessed by measurement of MCA-PSV at 1-2 weekly intervals starting from about 18 weeks. Cordocentesis, for measurement of fetal hemoglobin concentration, is carried out in those that develop ascites or a high MCA-PSV
